一方で、多くの人が聞いたことがあるであろう「老人ホーム」のように、自宅以外で高齢者の生活を支援する施設も存在し、家族が介護を行う負担の軽減を担っています。では、それらの老人福祉施設は、果たして高齢者社会に追いついているのでしょうか。本記事では八王子市においての老人福祉施設の実態を探っていこうと思います。

要介護認定とは？

要介護認定をご存じでしょうか。65歳の高齢者で、介護が必要と考えられる人なら誰でも申請することが可能であり、要介護認定を受けた人は介護保険によりさまざまなサービスを受けることが出来ます。老人ホームに入居したいと考えた場合にも、要介護認定の有無やレベルで選択肢が変わってきます。レベルは「要支援1-2」「要介護1-5」の計7段回です。要支援者は部分的に、要介護者は日常的に介護が必要な状態とされています。

上にあるボタンを押すと、各介護度の人たちが入れる施設の定員数のところに線がひかれます。右側の茶色いグラフが線を越えている場合は、施設定員数は足りていないということになります。要介護3-5を押してみましょう。介護認定を受けた人数が、線より下にあることが分かります。これは、特に特別養護老人ホームにおいて、入居できる条件を「要介護3以上」としているところがあるからだと考えられます。一方で要介護2以下の人たち、特に要介護1のところに注目してみましょう。認定を受けた人数に対して、入れる施設の定員数は明らかに足りていないことになります。\n

\nただしここで確認してもらいたいのは、このグラフには「特別養護老人ホーム」「介護付き有料老人ホーム」「住宅型有料老人ホーム」の3つしか含まれていないことです。他にある選択肢として、「サービス付き高齢者向け住宅」や「グループホーム」などがあります。また、原則として「要介護3以上」を条件としている施設でも、状況次第では要介護1の人が入居できる場合もあります。それらの定員も含めれば、もう少し入れる定員数多くなります。\n

\nしかしながら、施設の定員が足りていれば安心、というわけでもありません。お金の問題や地域の問題なども関わってくるため、全ての施設が選択できるわけではないからです。よって、含まれていない施設を考慮したとしても、この図が示す結果は少し心許ないと感じる方もいるのではないでしょうか。

特別養護老人ホームの費用

こちらは八王子市内の特別養護老人ホームの、月ごとの基本料金を並べた図になります。(情報が確認できなかった施設は含まれていません) 基本料金は「介護サービス費」「居住費」「食費」を足して算出しました。
要介護度が低い人ほど安く、高い人ほど高い費用になるため、このように料金に幅が出来ています。ここからは、月額10万円前後～17万円前後の施設が多いことが読み取れます。また、入居時に支払う「入居金」が0円であることも特徴です。

ただし、これはあくまでも基本料金であることに注意が必要です。特別養護老人ホームでは「加算」という、個人に合わせて提供するサービス費用があり、必要に応じてサービスをつけていくようになっています。また、日常生活用品費やレクリエーション費用は別途かかるため、実際に利用するときはこれより値段は高くなってきます。

逆に安くなるケースもあるかもしれません。介護保険には負担限度額認定というものが存在し、ある要件を満たせば「居住費」と「食費」を軽減することができます。負担限度額認定は、所得や預貯金などの経済的観点から判断され、認定者はさらに所得などの条件から4つの利用負担段階に分けられます。第1段階は最も負担が軽く、段階が上がるにつれて負担がおもくなっていきます。今回の図では一番負担の重い第4段階で料金を出しているため、より低い段階に当てはまる場合は基本料金はこの図より安くなります。(参考：【FPが答える】介護保険│負担限度額認定証で費用負担は軽くなる？)

こちらは、(介護付き、住宅型を合わせた)有料老人ホームの月額費用を高い順に並べたものです。こちらは八王子市が公表している情報をそのまま引用しており、介護度に応じた金額の違いは含まれていなかったため、このような表記になっています。実際には別途料金が発生することは特別養護老人ホームと同様です。こうしてみると、13万円前後～18万円前後の費用で、特別養護老人ホームと大差はないように見えます。

\nしかし、特別養護老人ホームとの最大の違いは、入居金が発生する場合があることです。入居金とは前払費用、入居準備金、敷金などを指します。マウスをそれぞれの円に載せてみると、月額料金が安いように見えても、多大な入居金が必要な施設もあることが分かると思います。\n
